Choose one fare

Select General NEMT, Procedure Ride and Return, Oncology & Infusion, or Rehabilitation & Therapy. Choose ambulatory or wheelchair transportation and 1, 3, 6, or 9 round trips.

02 · OPTIONAL SUPPORT

Add your extras

Additional loaded mileage, priority pickup requests, and reserved Wait & Stay are separate selections. Procedure Support includes its own reserved time.

03 · RESERVATION DETAILS

In checkout notes, enter your preferred appointment dates and times, pickup and destination addresses, return arrangements, and any requested additional stops.

Mileage is measured per leg.

The first 10 loaded miles on each scheduled one-way leg are included. Beginning at mile 11, additional loaded mileage is $4 per mile for ambulatory transportation or $5 per mile for wheelchair transportation. Use the confirmed chargeable mileage quantity for your route.

Choose one transportation fare or package and only the applicable extras. Wait & Stay is $130 for the two-hour minimum; additional escort time is $32.50 per 30 minutes. Priority requests under 24 hours are $15 when accepted. Additional stops are requested in notes and quoted separately before the reservation is confirmed.

Package terms before purchase

3-, 6-, and 9-round-trip packages expire 120 calendar days after purchase. Packages and unused credits are nonrefundable, subject to legally required exceptions and company-cancellation terms. Transfer to one named individual requires a written agreement with Loved & Heard and does not restart the expiration period. One extension of up to 30 additional calendar days may be granted by written agreement with Loved & Heard before the original expiration. The extension is not automatic. All rides must be completed by the agreed final deadline, no later than 150 calendar days after purchase. After that deadline, the package closes and all unused credits expire without a refund, subject to the stated legal and company-cancellation exceptions. Review the Refunds and Cancellations policy before paying.

Wait & Stay and Procedure Support provide a non-medical presence. They exclude nursing care, monitoring, medication assistance, lifting, transfers, and hands-on personal care. Procedure discharge requirements must be verified with the facility.
